Prairie Pointe to transition Rehabilitation Suites to 48 Assisted Living and Memory Care Suites

In Altoona, our local skilled nursing and rehabilitation provider Grace Lutheran Communities-Prairie Pointe and River Pines announces transitions of care on their Altoona campus. The transition kicked off in July and is anticipated to be completed this Fall. When complete Prairie Pointe will dedicate 48 rooms to provide Assisted Living and Memory Care services to Seniors. Skilled Nursing and rehabilitation services will continue to be offered on the campus at River Pines.

Grace Lutheran Communities CEO Amy Duhr said, “We are very excited about this change to provide additional services for seniors at our Prairie Pointe facility. We continue to focus on adapting to meet the diverse and changing needs within our surrounding community.”

This addition to the campus completes the continuum of care on our Altoona campus and allows us to use our expertise in rehabilitation to create a truly unique Assisted Living experience for our area.”
